Physical Examinations

Students registering for Athletics must have a current sports physical examination form on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the nurse no later than 3 school days prior to the first day of practice/tryouts.  Sports physical exam forms can be found on any athletic page of the GSL Website and also on this site under other forms.

Physicals are valid for 3 years  from the date of the last exam.  If you have questions of when your last physical date was - please call the GSL Activities Office at 320/864-2455 or e-mail Lori Harms at lharms@gsl.k12.mn.us     Thank you!
